Waiver Services

Supportive In Home Services:

In-Home Support services are residential services that take place in the individual’s home, family home, or community settings and typically supplement the primary care provided by the individual, family or other unpaid caregiver. Services are designed to ensure the health, safety and welfare of the individual and work on building skills. 

Community Coaching:

Community Coaching  services are designed for individuals who need one to one support in order to build a specific skill or set of skills to address a particular barrier(s) preventing a person from participating in activities of Community Engagement. 

Community Guide:

Community Guide is a short-term, highly personalized service provided by a qualified developmental disability professional (QDDP) trained in Person-Centered Training and Community Connections through DBHDS. It offers one-on-one consultations with Megan to help individuals explore community resources, identify interests, and connect with natural supports, thereby reducing reliance on paid services and encouraging independence. The program lasts 3–6 months and begins with initial interviews to assess support needs and develop tailored plans that can be adapted as needed. It is especially valuable for newcomers to the waiver program or those transitioning from school to adulthood. This service can start quickly and can be coordinated with in-home services while waiting for a DSP to be identified (separate from in-home services but complementary to them). The initial phase focuses on discovery, questioning, and exploring opportunities, followed by action-oriented efforts such as forming new clubs, joining organizations, learning to use public transportation, or improving social skills. The Community Guide supports individuals in overcoming barriers to full involvement in their community, such as a lack of transportation, by helping them learn to use the bus, access other transportation options, or seek appropriate services. While results may not always be immediately visible, positive outcomes—such as gaining independence and finding employment—have been observed. Success depends on individual motivation, resource availability, and personal responsibility, with the goal of fostering self-motivation and reducing dependency. Community Guide services are immediately available, and Community Guide housing services are being pursued.

Sponsored Residential:

Our Sponsored Residential homes provide a comfortable and supporting environment where individuals can make choices in activities of daily living and are professionally supported, along with their parents/guardians and their circle of support in planning their future. The person-centered planning process considers the person’s strengths, capabilities, preferences, lifestyle and cultural background. Through the provision of these services and supports, the person is provided with an environment that allows for maximum quality of life and growth through individual choices and life desires. SOS Sponsored Homes will focus on the individual and will promote quality and progress towards outcomes by addressing needs for choice, relationships, lifestyle, health, well-being, rights, and satisfaction of life.

Mental Health
Skill Building

 

  • Provides training in or reinforcement of functional skills and appropriate behavior related to health, safety, activities of daily living and access to community resources;
  • Supports medication management and related issues;
  • Monitors health both mental and physical;
  • Monitors nutrition and physical condition

Who Can Make a Referral?

  • Family Members

  • Individual Needing Support

Please fill out the form with as much information as possible and either email or fax to the closest office location.

Step Two

A representative from the program will contact you to review information and schedule a time to meet together. At that time we will gather additional information about service needs, scheduling, and preferences so that we can best serve you.

Step Three

The team will work on completing/reviewing assessments, creating a plan for support and schedule, reviewing staff options, and sending information for authorization of service.

“It’s hard to put into words how I feel about my “job” because it doesn’t feel like a job most days. When I head to my clients house I am not filled with dread like most jobs people have. I wonder what kind of day he has had and what kind of evening it will be.  I always have a plan but know if it has not been a good day we will let him lead what we do. Time is precious and that is what we do together is enjoy using our time to uplift his spirits while we teach each other life lessons.
